For five years I have been through nearly every organisation and process eg PALS, PHSO, CCG, CQC, Inquest process, trust CEO, Palliative experts and bodies, healthwatch and much more to try and get investigation, learning and robust changed behaviour based on undisputed huge suffering my mother was forced to endure due to multiple healthcare failing over her last three days of life.

Apart from the Royal college of anaesthetists responding to a report I paid for and patient safety dept in NHS England, and the patients association and AVMA, and a handful of often powerless individuals, all bodies have failed due to a mix of incompetence, lack of patient respect, or patronising tokenistic treatment, or too busy, or defensiveness, or more sinister or cynical denial and dissembling. Or ignoring serious safety issues, breaching legal obligations.

The system needs overhaul. The NHS is brilliant but must improve and needs more resources. I could provide evidence and explain all of this in more detail. The point is robust change.
