"No regular medication given to my elderly father"

My 91 year old father, who has altzeimers and takes a variety of medications including warfarin and donepezil , was admitted to a and e with chest pains on the night of 22 July. In a and e we gave staff a bag with all his regular medication plus a list of when he had to take them. I went with him when he was taken to AMU, and his medication together with a list of what he needed to take and when ,went with him. I left him at 4:00 am with the expecatation that he would be given his regular medication. He was discharged on 23 July and when we received the bag with his medication none of it had been dispensed to him. I am really concerned and dismayed by this. What did we have to do to ensure that this elderly altzeimers patient who has a pacemaker and is on warfarin received the medication he needs on a daily basis? I would be grateful if someone could contact me asap to let me know why this happened. It has had an adverse effect on my father's health and caused his family a great deal of distress.
