I have been admitted to LRI since 26/02/2015 due to having a fall as i was going upstairs at my house which caused a back injury. I was discharged 01/03/2015. Plus I had 2 falls while was in hospital. As i was in a lot of pain they decided to refer me to the Pain team who was supposed to see me in hospital on Monday 02/03/2015, however on Sunday 01/03/2015 one of the nurses and one of the doctors said that I will be discharged, I said that I am waiting to see the pain team, and they said that they will write to me as OT. Since the Discharge the pain has been increasing day after day. Now moving to the shocking discharge letter, I am still in shock since I have seen it: 1- The discharge letter never mentioned any thing about the 2 falls I had in hospital 2- most of my medication they got it wrong 3- No mention of referral to pain clinic 4-They did not provide me with 28 days medication as per policy, even with the medication I took with me 5- When I first admitted the bed was not changed from the previous patient, and I had to ask the family of the patient next to me to change it I am now admitted to LGH as a result of the pain that went way out of control and admitted as fail discharge
