When thinking about discharge you need to discuss plans with your clinical team explaining to them why you feel you are ready for discharge and what support would help you manage in the community. Your clinical team may not agree with you but they should be able to at explain what they feel you need to acheive before they are confident to put plans into action to allow you to return to the community. Weekly care planning takes place in named nurse sessions and in Ward rounds. Please make sure that you express your feelings in these forums. The patient Advoacy service can help you to get your point accross if you find this a challenge.

If discharge is on the horizon then planning usually happens within CPA meetings where the community team will be invited to help a patient prepare for community living. You also may have the right to apply for a Mental Health Review Tribunal. Please speak to a member of staff to discuss your rights under the Mental Health Act 1983 if you think this might be the case.
