"All staff were very kind and reassuring"

About: Grantham & District Hospital / General surgery Grantham & District Hospital / Gynaecology

(as the patient),

I had a LLERZ procedure in 2007 and the Mirena coil was not removed beforehand and the threads tucked up into my uterus and left in situ. Later they could not remove the coil as could not find the threads. It was then decided in 2018 to try and remove the coil by LA in clinic but due to cervix scenosis it was too painful and they could not gain entry so again the coil was left in situ.

I recently went for a GA Hysteroscopy as following a scan for a suspected cervical polyp it was found that I had fluid in my uterus and also the coil still there.

However the GA Hysteroscopy failed as the neck of my wound was tightly shut and no access and procedure aborted.

I am now awaiting a MRI scan to see if fluid appears sinister or safe to leave with coil in situ and then go back to the consultant to see next step.

All staff were very kind and reassuring and Mary who assisted me on the ward. All the very professional staff fully explained the procedure and checked all my ops constantly. I have no complaints at all - very caring.

I just wonder after everything I’ve been through really unnecessarily, if the original surgeon who did the LLETZ procedure in 2007 had of removed the coil before the treatment and then I wouldn’t have had to go through all this extra treatment?

Dear Nashiradra68,

Thank you for posting your response regarding your latest appointment at Grantham Hospital. I am so pleased you found all staff professional and caring. This will be passed onto the team.

In answer to your question regarding LLETZ:

Practice for LLETZ treatment in women with a Mirena IUS has not changed. The coil threads are tucked up into the canal of the cervix (not into the uterus) with the hope that the LLETZ can be performed and the threads retrieved from the endocervical canal after the procedure. Sometimes the threads are damaged by the diathermy but as they are still attached at a higher level can still usually be retrieved.

We may remove a Mirena IUS prior to LLETZ if there has been no unprotected sexual intercourse in the 7 days prior, if there is a risk the depth of excision may be compromised by the IUS or for individual preferences of the patient. I am not sure if any of these applied to you at the time.

Cervical stenosis is a known risk of LLETZ procedure as stated on your consent form. However removing abnormal cells is imperative to prevent the development of cervical cancer. It is likely the stenosis would have occurred with or without the Mirena IUS in situ.

Cervical Polyps can develop at any time and it's unfortunate that the cervical stenosis and inability to access the uterus has meant this is more difficult to manage.

I wish you well with your ongoing care.
