"Misdiagnosis, unsafe treatment, blocked communication, loss of trust"

(as a service user),

I received an inaccurate mental-health diagnosis and was prescribed medication that made things worse. Later assessments confirmed a different diagnosis. During this period my attempts to raise concerns and correct my record were blocked for over a year. I still struggle to get a safe, trauma-informed response and to have my data corrected. This has left me feeling unsafe and ignored. I want acknowledgement, accurate records, and assurance this won’t happen to others.

What could be improved? Accurate diagnosis, safe prescribing, open communication, accountability, trauma-informed care, better data handling.
