About: Intermediate and Transitional Care / Community Rehabilitation Team

(as a relative),

My mum fell at home and broke her hip and thigh bone, she needed help to get her back on her feet to live independently again. The rehabilitation team that came out to my mum after she came home from hospital after her surgery, they were absolutely amazing, they were so attentive to her and her needs to help her get back on her feet again, she really liked that they talked her through everything that she would be doing, they were kind, caring, professional and very pleasant and always made time for a chat with her, I can't thank them enough for what they have done for her.

Good afternoon Tine,

Thank you for taking the time to leave feedback for the Community Rehabilitation Team.

As the manager of the team, it heartens me to hear that both you and your mother had such a positive experience on her return home following her hospital stay. It pleases me to hear that your mother was kept informed of what staff were there to do and found them to be professional and caring during her time on the service.

I will be sure to pass on your kind words to the whole team - it will bring a smile to their faces (as it did mine).

I'm delighted to hear that your mother is back on her feet following her fall and wish her well as she continues her recovery.

Best wishes,

Lucy Jordan
