(as a carer),

My Father was in the final stages of his life with end stage kidney failure, it was really difficult to know what to expect and how to best manage him at home - my brother and I, alongside the incredible carers from NW Care and Glencaring care teams were keen to ensure he remained at home with his family.

We received an exceptional and responsive service from District Nursing and Rapid Response Nurses who were so compassionate and skilled in delivering excellent care at home during a really difficult and uncertain time. They provided us with practical equipment, guidance, endless support and in the moments around the passing of my dad - Jenna (district nurse) was brilliant - so kind and caring.

Given this support my brother and I were able to honour my dad's wishes and keep him at home. He passed away comfortable surrounded by his family and this really only possible due to the care and support from the nursing team. We extend our thanks and heart felt gratitude for the service the delivered up to, during and after this passing.

I would also like to thank the Renal Service for the monitoring of his kidney function and the support and service they provided in the years prior to my Dad's passing - this too was an excellent service.
