"Improvements Required"

About: Community Services (AMH) / Home Treatment

(as the patient),

I was placed into this team during a crisis episode in 2025. Every engagement I had raised further questions around the effectiveness of the care provided and created more issues for me until I requested a new care team.

I was provided with conflicting information, false information and a lack of medical support. I was offered some options of more intensive crisis support away from home and therapy I have already had, but as my needs are complex I couldn’t agree to this and for that reason. I was left to essentially resolve my issues on my own.

I was concerned about this happening to someone in a more vulnerable position than myself and raised the issue with PALS due to my concerns. Upon receiving the report, the documentation and information provided by the Home Treatment Team seemed protective of themselves and not the patient.

A combination of inaccuracies and untruths recorded on my medical records was sufficient enough for my legitimate concerns to be disregarded.

I am aware that I could challenge this further, but I don’t have the energy or capacity to do that, so I feel my only option now is to share my story with people potentially engaging with this service so that they know the risks involved. Major improvements are required.

Dear jiveej66

I am very sorry that this was your experience with our services. Our aim is always to provide compassionate and effective support to individuals as they navigate their recovery journey.

As an organisation, we offer treatments in line with NICE recommendations. While some people may already have received these, there are often additional modalities and pathways available within our therapy services.

Psychiatrists are an integral part of the multidisciplinary team within the Home Treatment Team. They meet daily and contribute to clinical reviews and discussions regarding all individuals on the caseload. A consultation with a consultant psychiatrist is not always conducted face to face — they may review records and information shared by the team and make clinical recommendations based on this. I understand this may cause some confusion, and I will ensure that our staff communicate this clearly to everyone under our care.

I would be happy to meet with you to discuss your concerns further.
