"Feel a lot more confident in my ability to be independent"

About: Intermediate and Transitional Care / Community Discharge Service

(as a service user),

I was discharged from hospital recently. After two weeks lying with very little movement and loss of muscle strength it was difficult to imagine how I would manage at home: everything was difficult - getting out of a chair/sofa, climbing the stairs, showering.

The assessment made by my OT - Emma - was efficient and she addressed all my needs in a very timely fashion; contact was made within 24 hrs.

She has a very professional and courteous manner and she also lifted my spirits as I could see how the aids that she would bring were going to help me.

The following day she came back with all the equipment we had discussed and, on reflection, when I needed some further items she came back a second time with those as well.

She has also arranged for grip handles to be installed in my en suite shower room and she also assessed my needs at the front door for a further grip handle and the best positioning for it.

Emma also assessed my ability to use the stairs and climb into and out of the bath and en-suite shower safely, as well as ensuring I could use the kitchen safely, so as a result I feel a lot more confident in my ability to be independent at home again and my quality of life will return to my baseline soon, I hope.

I feel the Community Discharge Service provided was timely, efficient, excellent and tailored to my specific needs and it was delivered in a caring and very respectful way.

I was also left with contact numbers should I need further help in the future, which was another reassurance for me.

Thank you so much for taking the time to write such a lovely review of Emma's input. Within Community Discharge Service, our aim is to respond as quickly as possible after discharge home as we recognise how being in hospital can reduce movement and confidence.

I am delighted the team responded so quickly and Emma was able to provide equipment that will help you be more independent at home and therefore help your confidence grow.

We aim to be person centred in our response and I am glad that you feel our input was tailored to what you needed. I am also proud that you felt the input was caring and respectful.

I will pass on your comments to Emma, and the wider team. Emma will be so pleased that she has made this difference to you.

I wish you all the best in your continued recovery and hope that you are back to your previous level of managing in no time at all.

Kind regards

Gillian McElhinney
