"Waiting on a questionnaire"

About: Community Services - North Devon / Bladder & Bowel Care

(as a service user),

You apparently sent me a questionnaire from the Bladder & Bowel Care Service which I have not received.

I do not understand how to use MY Care, which you may have sent it in. Please send it some other way as I do not use MY Care. I am worried as you say because I did not answer the questionnaire you have discharged me from the service.

I am sorry that you did not receive a questionnaire. Please get in touch with our department directly on 01392 208478 or rduh.bladderandbowel@nhs.net.

Our admin team will be more than happy to check your contact details and send a questionnaire for you to complete. The referral can then be reinstated.
