About: Countywide Community Care Service Delivery Unit / Community Stroke Service

(as the patient),

I suffered a stroke late July 2025 and was discharged from hospital 3 days later in the morning. That same afternoon a physiotherapist from the team visited me and put in train an exercise programme under the supervision of rehab nurses/ physio's. The plan worked really well. I had a phone call each weekend advising me of the upcoming week's schedule which, without fail was adhered to. I found the staff empathetic to my condition and vey encouraging regarding my progress. I really cannot fault the care and attention I have received from the team. Whilst I feel that I still have some way to go until I am have fully regained my strength and balance/mobility I am extremely grateful for the care and attention I have received.

Thank you so much for taking the time to post this. It means a lot to the team to hear about patient experience and this positive feedback will be so well received. It adds to their job satisfaction and makes them feel valued.

It sounds like you felt that you were well supported and that there was an efficient plan. I'm also really pleased to hear that staff were so good at being both empathetic and encouraging. I do believe that robust psychological support is equally as important as the actual therapy following a stroke which can be life-changing.
