"Life is so much better than it was"

About: South Eastern Health & Social Care Trust / Condition Management Programme

(as the patient),

Initially I self referred to the Condition Management Programme (CMP) from advice on work Dr as I had been suffering from multiple chronic conditions and  stress at home and work. I felt like there was no one or nowhere to turn to until I met the lovely people at CMP who gently guided me to improve my physical and mental health.

They taught me easy but effective exercises to relieve my sore stresses joints and muscles and how and when to fit them into daily routine. Worked a lot on calming my mind and pacing my life. It put my life in perspective and what mattered to me the most. I was given loads of information on tips and aids to help my recovery and most importantly I felt listened to and the sessions were geared to my specific needs.

I’m still a work in progress but I feel encouraged to take more steps and progress myself now. I was made to feel that my needs, wants and decisions should be a priority for me, and that it was OK for them to be different to the popular norms. I now go to yoga and tai chi classes and frequently go for a swim. None of which I did before. I’m building resilience and stressing less. Life is so much better than it was.

Thank you for taking the time to write a care opinion for our service, The Condition Management Programme in The South Eastern Trust. We appreciate you sharing your story from the self referral process to being a part of the Service itself and how much of an impact it has had on your life.

We have really valued reading your response and sharing it with the whole team. We work daily with clients like you who are living with long term conditions. We appreciate how difficult that can be.

I want to commend you for joining the Programme, engaging with our clinicians and how wonderful to read that with guidance, support, encouragement and education around your health conditions, you feel empowered to continue your efforts in managing your condition.

It is really valuable to engage in new activities such as the Tai Chi classes and swimming. We know this is not always easy. But I can sense by doing so, it has provided multiple benefits to your health and wellbeing and no doubt lots of but the enjoyment along the way.

I want to thank you again for being apart of the Programme and wish you all the best for the future.
