I am a dialysis patient at Antrim Area Hospital.
I am in the process of transferring to home dialysis (machine located at home).
I was given an application form for a rebate on electric I will use for the home dialysis machine.
I filled in the form and returned it with a bank statement (as requested on the form).
A week later the form was returned to me, with a note saying please complete the updated form.
I had been sent an old outdated form.
Which was useless and a waste of my time and money using to apply.
The second form I was sent now stated that I needed the bank to verify my details, verified and signed.
I have trouble waking and with great difficulty I went to my bank (25 miles distance) and got the form verified
I now have received an email saying that the bank employee who verified my details, isn't the correct person.
I am dealing with CKD, dialysis three times aweek, severe lymphoma in my legs, diabetes and retinopathy.
If I had know that to apply for the electric rebate, the form needed to be signed by my bank I would never had applied.
The difficulty in me getting to my bank is extreme, walking is very difficult.
My bank has informed me that I would need to visit a Belfast branch to get another new form verified, this would be impossible for me with my medical conditions.
The only reason I applied is because the first form I received stated that a bank statement could be enclosed with the form.
When the second form requested bank verification I made the effort to travel to my bank, arranging transport and with great physical difficulty proceeded in getting it verified and signed.
My point is why was I sent an outdated obsolete form?
Why does the HSC do this.
Instead of writing to me about outdated forms, the HSC should write to whoever sent me the outdated form in the first place.
