"Failure of my safety"

About: Radiography services / MRI Services

(as the patient),

I felt very uncomfortable in my experience and keep thinking what if and the risk I was put at and don't want anyone else ever to be at risk. The temporary hut is beside the MRI scanner outside the Daisy Hill hospital.

This worker who got me to fill out a form which is policy. There was a male waiting on his wife as she was in the scanner separate from hut.  I was then told to take off my bra behind behind a blue curtain that had a black bed there. The male who was a stranger to me was sitting reading a paper right beside the curtain.  The worker left the hut then, which shocked me! With a stranger, a male who he knew his wife was in scanner and he was a stranger to me, and I went behind curtain, slid off my bra under jumper and then I realised just from my own knowledge no metal allowed in MRI scanner! There was metal round on ties of my trousers, the worker did not state to me about any metal etc just my bra.

I felt uncomfortable that a stranger was behind me so I tried to keep eye on behind me. The worker did not come back til about 10 mins while I got changed into a cloak. This was very high risk, this person could have did anything to me as the worker went into the scanner and left me alone with this stranger as to take my bra off. There was an extra worker on that day in scanner so they weren't between jobs of MRIs.

When I got to scanner the same worker asked me what I was getting MRI for. I was shocked as they should have known by my referral. So I told them where I thought it was for as my unwell parts of the body. This was same worker who left me in temp hut with a stranger. So the other worker or they did the scans.

On my way out I seen computer where it stated my name and looked like a letter font I'm sure this was bound to have stated where to MRI.

As I got home I kept thinking about the failure of my safety and ever since this still think about it , getting undressed and left with a stranger even though blue curtain was there. Anything could have happened, it scares me to think and can't stop thinking how neglecting this was for a hosp worker to leave me with a stranger in a temp hut and get undressed and 10mins! The worker should have asked the stranger to leave to the reception in hospital, or stayed til I got undressed into the gown supervised til i had changed to ensure my safety under hospital standards.

I don't want this ever to happen to anyone else I was very lucky that day, the next one might not be.

Also, what would have happened if I had have wore my trousers with the metal on them? I've a pic of them, and also why did this same worker ask me what I was being referred for. I spoke to the referral about a worker asking me they said it stated on the letter on computer what needed MRI.

Please look into this and please ensure this never happens to anyone again being left unsupervised with a stranger to get dressed.  Also, it should be explained to the patient about metal in MRI scanner, not just take off your bra and put cloak on. What would have happened if my trousers where worn into the scanner?

Do these workers not read referral letters to know exactly what to MRI? Instead of asking a patient?

Good Afternoon Cathy44

Thank you so much for taking the time to share this feedback with us.

I want to offer an apology for the care you experienced during your recent visit to the mobile MRI unit. Specifically, I would like to sincerely apologise for the distress, discomfort, and indignity you experienced during your appointment.

Please be assured that the experience you described - particularly being asked to remove personal clothing in an inadequately private area falls significantly short of the high standards of dignity, privacy, and respect that we aim to provide for all our patients.

Standard protocol when the reception pod is unattended is for female patients to change into suitable attire on the mobile unit itself in the dedicated changing area behind a lockable door with lockable cupboards. Also standard protocol is for said patient to be fully changed and escorted off the trailer before the next patient (particularly a male) is brought onto the trailer. This is for many reasons including privacy and dignity, but also for the security of patient belongings.

I would also like to apologise that the communication with our staff did not fully address your concerns at the time. Although they were acting with good intent, it is clear that more sensitivity and privacy should have been afforded to you. As health care professionals we have a duty of candour to offer an apology and to assure the patient that their concerns are taken on board and will be shared with the team in order to continually improve our service provision.

It is recommended by the Society of Radiographers as part of pre procedure checklist to ask the patient what they believe they are having scanned so that the MRI referral is justified and not in error. This ensures that the radiographers are performing due diligence checks.

It is with this in mind that I want to thank you personally for bringing this to our attention. I will ensure that staff reflect and learn on this incident.

Dignity awareness is an essential element of person-centred care and something that is championed within the Southern Trust. Although it is disappointing to read your story it is also an opportunity for shared learning and to acknowledge that more needs to be done.

Update posted by Cathy44 (the patient)

The temporary hut had no lock door in the changing area. Just a blue curtain to pull over. I am not sure where you are supposed to get undressed.

There was nothing to separate other members of public waiting on there family members in same hut. I don't think it is ideal for members to sit and wait on family from scanner, and have changing area same place when it is only a blue curtain that slides across. And the worker left the hut with me not supervised for my safety. Anyone who could have walked into the hut. Not much privacy, and no lockers were even mentioned to me to store my belongings, I don't think there was any.
