This service is a must and is required after discharge until medications are correct and the patient has stabilised. The work undertaken by this team ensures patients do not return to hospital due to their being no half way assistance between discharge and outpatient appointments. This can be an incredibly frightening and difficult time following discharge when you are acutely ill.

This service is a vital lifeline and even more so for those who have nobody else to care for and support them. Thank you for all your help and support during what has been a very difficult time.
