"Transition from children services into adult"

(as a service user),

My daughter transitioned from children services into LD adult services. Social worker Dorota helped to build the relationship with myself and my daughter. Supported living accommodation was found for her to settle in. Social worker involving me with my daughter’s care which I’m happy to do that.

My name is Jacintah McCaffrey and I am Assistant Service Manager in the Adult Learning Disability Services in Belfast Trust. Transition pathway from Children with Disability services to the adult services is a stage in a young person's life with learning disabilities in which major changes occur. It can be a worrying stage for the young adult and their family. I am therefore delighted to hear that Dorota was of great help to you. Its encouraging for the staff to receive positive comments as it assures them that they are providing a compassionate and relationship based care. Many thanks for sharing your experience.
