The Reablement Team were first in contact with our mother last year. Naoise and others assessed, cared for and then discharged mum at the right time, getting back in touch at different points when she needed it.
They have now been looking after Mum more regularly over the last four or five months, assessing her needs and as before, they have been consistently outstanding. Ainé, Julie, Trudie, Callum, Lisa and Barbara and others who I haven't had the opportunity to meet but Mum always speaks so highly of.
They are always so professional, carefully assessing mum's ongoing and acute needs, but also gentle, personable, caring and engaging with Mum. This has as much of an impact on Mum who is deeply appreciative of their support, as are we.
With other patients to see, we know the time they can spend with Mum is limited. Nevertheless, they are very observational and proactive, engaging additional services including the district nurse, emergency out of hours doctor prescriptions, her GP, day centre services and more. They are excellent at keeping in communication with us as a family but also keeping mum involved in decision making about her care and respecting her own efforts to stay well and independent.
This week our mother had a fall and Julie arrived, assessed the situation, called paramedics and stayed with mum, stemming the blood flow from her head injury for two hours until one of our siblings was able to get up from Dublin. We cannot thank her enough and are sorry her other patients will have missed out.
Overall this is an outstanding health service team. We are very grateful for everything they have done.
