My father who had dementia, passed away last week after a relatively sudden decline in his health. We were able to keep him at home, comfortable and peaceful surrounded by those who loved him very much.
This was in large part due to the care of the acute care at home team in the whsct who were fantastic. Their professionalism, knowledge and quiet compassion was apparent. The team have respect for each other and the handovers and continuation of care was seamless. A well oiled machine.
Mr Todd and his team are excellent and helped my mum and us gently through each stage. We cannot thank this team enough. This is the way forward and this model of care is definately worth investing in.
