(as the patient),

Had appointment with Tessa recently for help with swallowing problems with liquids. NHS have been thorough in their investigations and I have attended several procedures, all of which have given negative results.

I learnt a great deal from my session with Tessa and would like to pass on my thanks for her ability to convey what exactly is happening with my swallowing difficulties which were conducted in a relaxed and unhurried manner, many thanks.
