"My current care concerns"

About: Community Services - Eastern / Speech and Language Therapy Royal Devon & Exeter Hospital (Wonford) / Ophthalmology

(as the patient),

As it stands I have not had any contact with either consultant for quite some time.

I was seeing one consultant for my eyes - 

oculoplastic dept but over a year ago they handed me over to a other consultant. 

On the 'My Care' system all the notes are there and are very detailed including the Nufield notes of my MRI scan.

I have recently been discharged by my consultant from the Oakhampton speech therapy department as they couldn't do anymore.

Both consultants had actions to follow up regarding my sessions and was intending to see if I needed further botox.

Last contact was in March.

I really could do with a catch up, feel a bit in limbo.
