"Unable to get an appointment"

Doctors are running multiple clinics, but I feel they are not servicing patients effectively even in one clinic. Why are NHS doctors allowed to spread themselves so thin? 

My clinic asks me to call at 8 am for an appointment but when I call, the line is engaged and by the time I get through, no appointments are available and I can also not book in advance. If I go in person, they ask me to call or say the appointments are filled.

I am in my 80s with diagnosed heart failure and I have not been able to be seen by my doctor to date. The receptionist says to call 111 if there is an issue. The whole purpose of a GP is preventative and proactive  measures. Sounds like the current NHS practice works on emergencies only. 

As a result of years of absent GP care, I ended up in emergent care at Glenfields Hospital which provided wonderful care, but no follow up care is available to me and my GP practice has me running around in circles without an appointment.
