My relative was transferred from Lincoln County to Archer Ward after falling and breaking hip. After approximately a month on Archer Ward was discharged home. On arrival home was told the someone would come to assess what care she needed at home.
As soon as they came through the door they said relative should never have been discharged. Could not get in or out of bed on own plus other things person was not happy with.
They phoned Archer Ward to try and get relative readmitted as they deemed this an unsafe discharge. Nurse on phone said there was nothing they could do now relative had been discharged. The only alternative was to find a care home that had space for a temporary stay.
In the end we managed to find one, my relative had to pay a total of over £7500 for 5 weeks care until she was fit enough to manage at home. The after care/ care in the community is shambolic and not fit for purpose. If you have funds to pay no one is interested in helping find the necessary services need to help at home.
After working in the NHS for 22 years I am totally disgusted with the care on discharge and I am now embarrassed to say that I actually worked for the NHS. We complained to PALS… again total waste of time.
Just hope we never need to rely on these people again.
