Every outpatient or day surgery or inpatient contact I am asked to fill in a form on the appointment or admissions sheet giving GP name, my phone number, NOK etc. I have done this for urology, endoscopy, neurology, orthotics, fracture clinic and ophthalmology for both of us for three years, maybe  ten to twenty times with repeat appointments and never once has the information changed. I don think any data clerk is allocated. So if I cannot speak all this information is wrong and it will not be possible to contact relatives.

We sincerely appreciate you bringing this concern to our attention. We understand how frustrating it must be to repeatedly provide the same information across multiple appointments, and we recognise the potential risks this could pose if you're unable to communicate.

To help us look into this matter further and ensure your concerns are addressed appropriately, we kindly ask that you contact our Patient Advice and Liaison Service (PALS) team via email at rsc-tr.pals@nhs.net or by telephone on 01483 402757. They are best placed to support you and investigate your individual case.

In the meantime, I will forward your feedback to our Patient Services Manager so we can review our current procedures and explore ways to improve the consistency and efficiency of our data collection processes.
