About: Intermediate and Transitional Care / Community Discharge Service

(as a relative),

Matty & PJ (Patrick) and their student Fiona helped my mother in law for her transition from hospital to home. They were extremely friendly, sensitive and informative. They gave us and my mother in law the skills we needed to settle back at home and gain confidence after a terminal cancer diagnosis. They provided us with the apparatus we needed and helped us feel secure in our new situation.

Thank you so much for taking the time to share your thoughts on our team during what, I am sure, is a turbulent time for you and your family. I am so glad that PJ, Matty and Fiona were able to help in the transition from hospital to home and did so in such a kind way.

Although I am sure you and your family have a lot ahead, I wish you well in navigating any changes.

I will pass your kind thoughts to the team, who will be delighted to know their input made a positive impact at a worrying time.

Gillian McElhinney
