"Age related memory loss and patient app"

Today for a period of a couple of hours i could not recall my car registration, this kind of thing can happen at any age of course. I wanted to make a note of this in a place or way that was accessible.

The point im trying to make is that my chart would be the obvious place to record this type of thing and unless im mistaken there is no provision for patient entries or data.

Would a place for patients to communicate their experiences to medical professionals when the patient may not have the ability to communicate be a useful thing?
