"Ignoring my request "

I was told to do my complaint form again as I’ve mentioned someone’s name (can’t even take the blame!) I was asked to provide evidence that I went to therapy and I asked my GP for those documents about two months ago and they are ignoring my request I tried contacting the desk the chat doc and even booked an appointment to discuss about it too this is outrageous as I need the help to see if it will benefit me as nothings helping with my anxiety and depression so once again I’m filling this complaint form out to hopefully let the public see this and ask help from elsewhere it’s a joke even when a third party emails your GP for you
