"A lack of discussion with the family"

About: Borders General Hospital / General Medicine

(as a relative),

My Father was discharged from ward 6.  We appreciate the hospital is extremely busy, and staff gave the best care they possibly could given how stretched they were.  However, we were told they would look at referring my father onto the hospital to home team but he has been discharged home without any further care.  This available support was mentioned by a couple of nursing staff.

He is an elderly man living alone and has already experienced confusion managing his medication.  We feel that consideration for managing areas of daily living has not been fully considered prior to discharge.  Particularly management of medication.

Again we are thankful to the staff and feel the high volume of workload has prevented further discussion with the family regarding the hospital to home team.

Im sorry to hear of the poor communication experienced upon Discharge from Ward 6. If you would like to give us further information please contact the patient experience team. I am glad to hear that you experienced great care in ward 6.

I will pass this over to the Senior Charge Nurse to remind the team of the need for better communication.
