Could you please email a letter of consent form so that I can complete and return it. The text from the practice dated 5th June does not allow me to print it out from my phone.
Apparently I need this form to be able to request a letter for my cruise line that confirms that for age reasons it is not desirable for me to have a Yellow Fever vaccination. I have been talking to a lead nurse about this and about other vaccinations: typhoid and tetanus. The letter will be recognised by the cruise line as a letter of exemption.
The form as an email attachment please will fix the issue on my technology and then I can get it back to the practice
