"Lost paperwork"

About: Forth Valley Royal Hospital / Accident & emergency

(as a parent/guardian),

I am writing to raise a concern regarding the care my daughter received at FVRH Accident & Emergency department.

We arrived at A&E at approximately 23:15hrs because my daughter was experiencing chest pain. 

After repeated inquiries, we were informed that her paperwork had been misplaced, which led to the significant delay in her being triaged and treated.

This not only caused a great deal of distress and anxiety for both of us but also posed a potential risk to her health, given the nature of her symptoms.

I understand that emergency departments are often under pressure, but the loss of patient records and the resulting delay in care is unacceptable. I would appreciate an explanation of how this occurred and what steps will be taken to prevent this from happening to other patients in the future.
