(as the patient),

From childhood and abuse and this resulted in depressive state which required input form mental health service

Experienced the death of my husband my mental health required ongoing support via my CPN and G.P

I feel that CPN went above and beyond to support me and i have trust in my CPN that I could be open and honest about my feelings I knew that she would be there at the end of the phone if needed to speak to her outwith appointments she always made me feel normal and accepted

I did not feel that that there could have any improvements
