"Very friendly, very professional and very approachable"

About: Community Services (Adult Physical Health) / Community Palliative Service

(as the patient),

I have been given weeks left to live and received a home visit from a palliative care nurse recently.

Very friendly, very professional and very approachable

We discussed the situation and how the team can help me over the next few weeks.

She also sorted out a problem I had with my medication

Thank you for taking the time to tell us about your recent experience of the Community Palliative Care Team at a difficult time for yourself.

We really value feedback about the team as it can help shape our future services and also provide assurance and so was pleased to know you felt at ease, listened to and supported from your palliative nurse visit. Your feedback will be shared with the team including the nurse that visited you.

I am confident the team will continue to provide the same level of support and care in future visits to yourself.

Thank you again for sharing your opinion of our Community Palliative Care Team.
