"My father's discharge"

My father was discharged after a 3 night stay. I was so angry and upset when I realised that the decision was made to give my father money for a bus, sending him home in is pyjamas and slippers, when is it ever acceptable to send a vulnerable elderly man home on a bus in his nightwear?

He has cognitive issues and I doubt he would ever have made it home. His phone was stolen in the hospital so he couldn't make contact with us. My mother arrived in CAU to visit and was informed he had been moved to the discharge lounge. She is on oxygen and struggled to walk to the lounge when she then called me. 

He had no information about what the diagnosis was, what the treatment plan was, no medication or advice.  I followed this up and medications were sent out in a taxi that night and then again the following morning.

Flabbergasted that it was deemed OK to send him home in a bus but that medication could be taxied out, I was honestly lost for words! 

He is home now and settling and we are clear about what was wrong and the treatment plan but this was a terribly frightening experience.

Thank you for taking the time to share with us your father's experience. May I take this opportunity to apologise for your father's experience and for any distress this may have caused to you and your family.

It is always difficult for our staff to hear that we have not met the needs and expectations of our patients, as it is never our intention to cause any additional stress or concern. We would really like to be given the opportunity to look further into your concerns and to do this we need some further information.
