About: Primary Care / Reablement Service Promoting Well Being / Access and information service

(as a staff member posting for a carer/relative),

I contacted Access  & Information and spoke with Leanne for help and advice.

I was concerned about my Uncles ability to cope at home.  Options discussed in relation to package of care.  Referral placed to Reablement team.  I wanted to thank Leanne for the manner in which the referral was taken and discussing the options. I am very thankful and really appreciate the fast action of the service and how quickly the Reablement team attended to complete the assessment.
