"Appointment for a hormonal coil"

About: Sexual Health Services - South Staffordshire (Open Clinic)

(as a service user),

Got to the appointment after travelling 1 hour and 15 minutes.

Clinic empty and no patients to be seen, around 5 members of staff seen to be on duty but with no patients.

I completed checks such as BP, pregnancy test and answered questions.

I was then told I cannot have a coil today as they are only doing implants.

When I asked why they was only doing implants today they replied they just are.

I found it hard to digest that I could not have a coil when there was so many staff present with no patients.

I was told to rebook for another day which the next appointment would be in 6-8 weeks time, and told I must take the pill for those 6-8 weeks.

I replied I cannot take the pill due to migraines and being on mounjaro as advised by GP,

The nurse said they may not fit the coil at my next appointment if I haven’t been on the pill.

I explained my need for the coil due to heavy painful periods/ ovulation effecting my work and social life. I also have query endometriosis and ovarian cysts.

Left the clinic disappointed, confused and frustrated after waiting 8 weeks for my appointment.

Hi Unknown12345

My name is Suzie Sellberg and I am one of the Nurse Team Leaders in South Staffs Open Clinic services. I am not sure which clinic you attended and I am very sorry to read of your disappointment with the service you received. I appreciate your concerns about your migraines and Mounjaro medication affecting your contraceptive choices, but it is very unusual to hear that you have not been offered a fuller explanation and advice in relation to your questions. However, without further information I can't investigate this further.

To give some explanations regarding coil fittings: Any patient requiring a coil fit must have a full history taken prior to being booked for the procedure so that we can ensure the coil is safe and appropriate. We also need to make sure that a clinician trained in coil fitting carries out the procedure (not all of our staff are trained coil fitters because we provide multiple other services in our clinics so need a mix of staff who are skilled in different areas). It is extremely important to exclude pregnancy, especially if there has been any unprotected sex in the run up to the procedure because fitting a coil in anyone who could be pregnant is to be avoided at all costs. Also, as we are a sexual health service providing contraception, we are not commissioned to fit hormonal coils e.g. Mirena, to manage heavy painful periods and endometriosis.

Please accept my sincerest apologies that your expectations were not able to be met on this occasion. Please do not hesitate to contact me if you wish to discuss this further: suzanne.sellberg@mpft.nhs.uk. I will see what I can then do to provide some more detailed explanations and a way forward for you.
