I can only comment on my brief but welcome experience of health and care which followed a 3 day spell in hospital at the end of last year.
Hospitalised as I could not get back on my feet from a sitting position. I had been ok before this. It is possible that visiting may also have been the result of previous referment as a result of falls.
Following discharge from hospital, I was visited by a H&C Coordinator who advised me that I would be visited on 3 days per week and given advice and a recovery programme of exercises.
I was visited by cheerful and helpful care workers who gave advice where considered necessary. A second stage programme followed as it was considered that I had made good progress. I was then visited by the physio who after some tests, discharged me. The first week of visits also included ability around the house, going in and out of bed etc, no probs. 5*
