About: Mental Health Community Services / Condition Management Programme

Was at an all time low point in my life, my work was affected and I found myself having to take a career break to prevent loss of job. Whilst waiting on psychological services through NHS, (3year wait) my mental health social worker told me about the trusts condition management programme. I enlisted for their assistance and found they were quick to assist. I received help on a one to one basis from a mental health professional. This was vital on my road to recovery and helped me identify triggers and how to manage my anxiety. They worked in partnership with my social worker and GP. I am now back in work and know that if needed I can ask again for their assistance.
