"Poor communication"

About: Alexandra Hospital / Older people's healthcare Evesham Community Hospital Princess of Wales Community Hospital / Lickey Ward Worcestershire Royal Hospital / Older people's healthcare

(as a relative),

My mum's partner fell ill unexpectedly and was found to have cancer and mental capacity was seriously diminished. They were bounced around every hospital going, all of whom seemed desperate to discharge, although it was unsafe to do so. Never saw the same staff on and barely any communication on their condition/prognosis. We visited most days and watched them literally waste away over 3 months. No one had even bothered to let us know they were actively dying and we had visited by the Grace of God half an hour before they passed to hold their hand.
