"Lack of follow up"

Have mixed feelings about the recent experiences with this hospital and care

Family member Seen 5 weeks ago in outpatients, the consultant was writing to GP.  5 weeks later there has been no report to the GP surgery.  This timescale is not acceptable.  As part of this report the consultant was suppose to be mentioning about medication, so patient could access if needed.  But 5 weeks later nothing. If this is timescale of reports then there are serious issues, people's lives depend on communication with all professionals. This is not acceptable.

The consultant themselves could not fault in manner in which dealt with family member with care compassion.  However there is a lack of follow up. Family members health has been deteriorating and consultant is aware and there is no support and guidance for patient or family.  Left trying to navigate an awful life limiting illness on own. 

The time between appointments is too long, told a few months and it actually 6 months.  When NICE guidance states in patients deteriorating it should be 3 months.  Guidelines also says family should have access palliative care early on, but there is zero support and it is getting more difficult to get patient to appointments.  The timescale between monitoring is poor for such a horrible illness for patient and their family watching.  

Disappointed at the lack of care and lack of basic communication from hospital to primary care.  Exactly same last year, took 6 weeks or longer for report to be written. Seen massive decline in this area in last 12 months.  As prior this reports faster flowing.  Action needs be taken as this lack of prompt communication is vital to patient healthcare and delays like this can have consequences.
