About: Community Services - North Devon / Home from hospital

(as the patient),

It has been most helpful having the medication chart for the next 9 days. This details what tablets to take and at what time during the day. Also when to stop taking them. The only thing I had to search for was the correct phone number to ask for advice which was on another sheet. Maybe better to be on the medication sheet.
