(as the patient),

I wasn't well and ended up in hospital, kept getting switched to different departments and wards.  Then i had a fall and ended up with a broken left hip.  Had a brace and ball fitted which was good then transferred to a nursing home for rehabilitation, that was really excellent.

Now I am home, and glad to be home.  Not everything is sorted, still have problems, can't bend my toes.  They send carers in 3 times a day but I like my independence.  The girls are good but I can dress myself, wash myself, cook my food, but they come in and do that. I have a Zimmer frame so I can get around.  Like I said, I prefer my home and independence, but am grateful for what they have done.

My name is Ann Marie Conlon. I am the Falls Integrated Pathway Co-ordinator in the Western Trust. I am sorry to read that you had a fall where you sustained a hip fracture.

I am pleased to see that surgery went well, and that you benefited from the rehabilitation that you received in both the care home and at home.

Hello,

My name is Linda Lyttle, I'm Head of Service for Intermediate Care Pathways and Recovery Service in Northern HSCT. It sounds like it may have been staff from our Recovery Service who visited you for rehabilitation in the care home and I'm glad to hear your positive feedback from this, following no doubt a difficult time for you. Thank you for sharing about your experience, the team love to hear how service users are getting on once they leave their care. I'm not sure if you are having any further rehabilitation now you have returned home, if you are not and feel this may be required please discuss this with your GP who can refer you to the appropriate service or team depending on where you live. I wish you all the best on your continued recovery.
