"The support provided was absolutely outstanding"

About: Clydesdale Locality Support Service / Home Assessment Team Lanark ICST (Community Nursing, Physiotherapy, Occupational Therapy) / ICST (Community Nursing, Physiotherapy, Occupational Therapy) Clydesdale

(as a relative),

Just after Christmas my Mother took unwell, post 2 breast Cancer surgeries and Radiotherapy, she was in so much pain she almost was unable to walk, up most of the night struggling to cope with the pain in her legs requiring 121 Family care 100% of the time.

Post a visit to A&E on New Years Eve - the support the home nursing team provided was absolutely outstanding, this care then moved to Social work and Occupational Therapy (Lee specifically) who also assisted in getting Mum back to being independent in her own home. No words can convey how much my entire family are indebted to all of this Team!!!! Thank you very much indeed!!!!!

Thank you for taking the time to share your experiences at what must have been a traumatic time in your family life, made all the more pressing at that time of the year. It is doubly satisfying therefore to hear that your mother's transition from hospital back to her home setting was achieved with the right care and support being provided at the right time and by the right people, enabling her to get back to being independent at home.

From what you describe, this appears to have eased both the associated demands and concerns of the wider family also. Hearing this outcome and your experience is very rewarding to all staff involved in providing their care and support role within integrated services. We will ensure that the associated staff and teams are therefore made aware of your kind comments.

Hope that your mother's recovery and independence has continued to be as optimal as it can be since coming home.

Thank you for taking the time to share your story and experience and I am delighted to hear the positive impact the various teams input have been able to provide.

Supporting individuals to maximise their independence, choice and control which remaining at home is a priority of South Lanarkshire Council and its health and social care partners and I am thrilled to hear that this approach has made such a difference to your and your mother during a very difficult period.

Kindest Regards,

Danielle
