(as a relative),

I was unaware of the existence of the Admissions Avoidance Team and was a little sceptical when mum’s doctor referred us to them. They were amazing! I cannot express how fantastic every single one of the carers, ANPs and district nurses were. So kind and caring to us all during a very difficult period of only a few days ( although it seemed much longer), which resulted in mum’s death. She ended her 103 years on this earth feeling loved and comforted. You have my heartfelt  gratitude. Thank you to you all.
