"Elderly mother discharged without consultation"

About: King's Mill Hospital / Older people's healthcare

(as a service user),

I arrived to visit my mother on ward C51 in mid-January to be told she was being discharged with no consultation with myself.

The nurse just handed over the discharge letter which had 2 things which were wrong. My mum did not have a fall and does not use a walker at home and in the state she was discharged cannot manage without help. No care plan arrangements.

And guess what she was readmitted yesterday.

Think there is a little confusion. It was ward 51 at King's Mill hospital!
