"Poor practice observed"

About: Daisy Hill Hospital / Stroke Unit (Level 4)

(as a relative),

A relative of mine was an inpatient on Level 4 in Daisy Hill Hospital. A few things were observed: 

My relative missed a dose of her antibiotic. It was meant to be administered in the morning. I asked the nurse about it when up visiting in the afternoon. The nurse administered antibiotic in the afternoon and told my relative to set an alarm to take the second dose in the middle of night at home.

My relative asked for paracetamol at 10am. Nurse told her she would give it to her at lunch. By 2.30-3.15, still no paracetamol and my relative asked again. The nurse said that the kardex was down in pharmacy. The nurse proceeded to pull out a strip out of her pocket and gave my relative 2 tablets. 

My relative was on IV antibiotics during her stay. At one point these were left in a tray which was  sitting on top of the clinical bin. They were left unattended for approx 30 mins while the nurse looked for a IV drip machine.

Many thanks for taking the time to highlight your concerns following your relatives recent stay in Stroke and Rehabilitation Ward, DHH and I would like to apologize that you and your relative had a negative experience. This is not the standard of care that we strive for in the Southern Health and Social Care Trust. I will ensure that the issues that you highlighted are raised with all staff at our daily Patient Safety Briefings in order for staff to learn from this. I hope that your relative is recovering well.
