Early january 25 my elderly mother was discharged from QMC Nottingham after a fall at over christmas followed by a part hip operation and a 2 week stay on ward F18.
It was apparent when my husband and I collected her and arrived at the family home, a mistake had happened as she was unable to walk even aided by the wheeled walker and assistance from us both. No strength or ability and was carried into the family home by my husband and put in a chair.
The care package in place started at tea time but the carers were unable to assist as they couldn't lift or carry mum. We had help from the community nurse when she arrived to give mum an injection and reached out to the urgent response team. As a family with my 2 brothers we had to manage mum that night.
They were unable to come out that night but came the following day, in the morning, and we discussed the situation in depth and Kate and Sue then advised a hospital readmission was the best option and progressed that for us, to which we were very grateful, as we didnt know what to do and the nhs social service contact Gary spoke with Kate when he called me back. It was such a relief someone had listened and helped, due to the stress it has bought on
