"Transition made simple, easy and reassuring"

About: Bassetlaw Hospital / Neurology Children & Young People Physical Health / Children's Epilepsy Bassetlaw

(as a parent/guardian),

My son has/is transitioning through many different services....and it has been far from smooth/easy/simple....apart from The Epilepsy Nurses.

Gemma is our Children's Epilepsy Nurse. Knowing how complex my son is and knowing the transition to adult services would be very complicated with  a lot of information to exchange, she set up an introduction to adult services. Myself, Gemma and Bev met a few times (once with my son) to discuss my son and his varying issues so Bev at least had a basic knowledge of everything that's occurred over many years....which would have been a huge task for her to undertake had Gemma just dropped us.

We currently act as a team of 4....myself, the neurologist, Gemma and Bev...we are all tagged in correspondence so everyone is aware of what is happening ready for the actual handover to adult neurology.

It's been brilliant....and a huge success! I believe both Gemma and Bev have found this to be very beneficial.....and myself as a single parent thinks it's amazing....it's put my mind at rest Bev being involved from such an early start...I wish all departments would show the same initiative!

Big thank you to Gemma for making this so simple, easy and most of all reassuring.

Good Morning,

It is fabulous to hear how beneficial Gemma's approach to a supportive transition process has been for you and your son. We understand how difficult and daunting transition between services can be and I was so pleased to here that this is enabling a smoother journey for you.

Thank you so much for taking the time to reflect and write about this, your feedback will support the continuation of transition clinics and these supportive approaches.
