"Discharges have been very poor"

About: University Hospital Monklands / Medicine for the Older Adult (Wards 14,20,22) University Hospital Monklands / Stroke/Medicine for the Older Adult

(as a carer),

Elderly relative has had recently had number of admissions to ward 22 and 20 monklands. Discharges have been very poor with support on discharge being inadequate. After difficult discussions have learned it's senior nurses/ discharges coordinators that do homecare assessments, albeit very poorly.

Conversations with facilitators have been confrontational, stressful and lack of empathy. Relative has got home on 3 occasions recently, assessments have been inaccurate and very quickly homecare manager has needed to increase care package.

Why is it a senior nurse and not hospital social work that does a homecare assessment. I think surely it is more cost effective and much more quality assessments to have a social worker/ social work assistant to do the "social care" assessment rather than the cost for a senior nurse who doesn't seem to have knowledge and understanding of social care.

Understand the process of integration but don't find the nurses on discharge to have any great understanding of a good, needs led, homecare package.

Hello Ann Frances.

My dad says thanks a lot for coming back so quick. We had a chat and as hes been in and out a few times and could end up back in he doesn't want to share his name.

Dad asks it is possible to review and assess general complaints about if nurses are best people to assess for homecare or is social workers/ social care staff better for this role and to post response on here

Hello, any updates on investigation and working out about nurses assessing for homecare and not the social work team? Surely the training, knowledge and understanding for social work team means they are much better place to assess for homecare rather than discharge nurses

Sorry to see there still hasn't been an update.

Relative has had further admission with similar experience which was discharge nurses who dont appear to have much experience with social work telling us what homecare dad would get. Why are nurses and not social workers assessing for social care

Dear B Smit,

apologies for the delay in the response to you, this was unfortunately due to reasons outwith my control. In relation to your query I can advise that review, developments and improvements around discharge planning and processes remain a high priority for the site and are constantly being worked on by both UHM and Social Work teams.

As I am sure you are aware there is lots of challenges around making changes to this process that are dependent upon resource, experience, capacity and demand but we are committed to consistently improving processes to ensure the discharge process works in an optimal way wherever possible. Sorry to not be able to give a more specific information in relation to your dads case but please be assured the work is ongoing.
