"Referred to physio for clavicle pain"

(as the patient),

I was in a car accident a couple of years ago and thereafter always had a little bit of pain.

I went backwards and forwards to the doctor and had a couple of scans and consequently found out that I have arthritis and my doctor referred me to the NHS physio.

Hello Allanh, I am sorry to hear about your accident and the subsequent pain and the delay you have had for getting the right help. I am pleased to hear that you have been referred to physiotherapy, and I hope that our services are able to support you once you are seen. There is a waiting list, however your referral will be looked at and a decision made as to how soon we need to see you based on the information we have, and we will be in touch with you to offer you an appointment once you are near the top of the waiting list.
