"Alzheimers Diagnosis and support after care"

About: General practices in Tayside Older People's Mental Health / South Perthshire Older People's Community Mental Health Team

(as a relative),

Following a few years of uncertainty and suspicions, although it was a difficult diagnosis for Mum to deal with I can't speak highly enough of the care professionals who have worked with us and supported us so far with the early stages of Alzheimer's.

From the care and patience shown by the GP in Crieffs Blue Practice to the consultants Dr Graham, Dr Cruz and most recently Dr Amin, their care, guidance and professional attention they have provided us with have been second to none.

Our appointments have been consultative and informative at all times despite seeing different professionals there seemed to be a joined up and holistic approach. 

Following these appointment the care, support and information given to us from the post diagnosis team has been timely, supportive and helpful.  In particular as a carer I have felt the support given supported Abbie has been excellent not only for Mum but and I feel I can approach her with any questions at anytime to help me support my Mum through this challenging time.

Abbie has given us suggestions with practical solutions, guidance in areas such as attendance allowance and solutions/support in teething problems with new medication.

I have found this support invaluable and it has given me the confidence to deal with a few challenging situations in the past few months,

Just knowing that there is a team of professionals there who have the same goals and can guide you through these uncertain times is fabulous.

The South Perthshire Older Peoples mental health team are a great example of how the NHS is delivering a superb service on a tight budget.  

I have nothing but praise for the experience we have had to date from everyone in this large team, the colleagues at reception who keep you informed and make you welcome, to the procedure which ensures the medication prescribed is delivered a few days later to Mum's home.

A huge heartfelt thanks to all involved for being there and hearing us.

Thank you so very much for taking the time to provide feedback on the care and support that you and your mother have received, both prior to and following her diagnosis of dementia. I will share your experience with Abbie and our wider team, including our colleagues in Primary Care, Medical and Nursing staff. I am pleased to read that your experience has been such a positive one and demonstrates the person centred care that we aim to deliver at all times.
